Medical Case Manager

The AIDS Coalition of Southern New Jersey has an immediate opening for the position of Medical Case Manager to provide case management services to HIV/AIDS clients.  Services include intakes, writing service care plans, referring and linking clients to in-house and other services, following up on referrals, accompanying clients to various appointments if needed, conducting home visits if needed, entering data into the computer system, being onsite at local clinics/hospitals, and providing on-call telephone coverage several times a week at the Bellmawr location.

Qualifications: Bachelor degree required; case management or other relevant social service experience required; experience working with mentally-ill, substance abusers, incarcerated and knowledge of HIV/AIDS preferred; ability to relate to individuals from various socio-economic backgrounds and to relate to clients in a culturally and linguistically sensitive manner is a must. Valid driver’s license and access to a vehicle is required.
