Medical Case Management
Medical Case Management is the very core of ACSNJ’s programs and services. Its goal is to identify areas of client need and, through a system of referrals, link clients to the resouces or services required. Our experienced Medical Case Managers are the conduit that provides access for our clients to essential medical care, medication and support services (such as mental health or substance abuse treatment, transportation, food banks and legal services).
Following a brief intake, clients are assigned to a Medical Case Manager who will contact them for an assessment. During the assessment, the Medical Case Manager will work with the client to develop an individualized service care plan and track the client’s progress toward meeting established goals. In addition, from 11 a.m. to 2:30 p.m. an on-call Medical Case Manager is available by telephone to handle clients’ needs if their assigned Medical Case Manager is not available.
