Care Outreach

Advances in medicine have prolonged the lives of those diagnosed HIV+.  Regularly scheduled medical appointments, including lab work, with an HIV medical provider allows for more accurate monitoring of your health, prevents minor health issues from growing into bigger ones, and helps you better understand what to expect when living with HIV.  We recognize however that not everyone goes to see their HIV healthcare provider because sometimes life’s ups and downs can interrupt this process.   ACSNJ’s Care Outreach staff applies a non-judgmental approach, understanding, and genuine concern as they work with HIV+ individuals who have dropped out of life-saving HIV medical care and help reconnect them with treatment.  Our goal is to locate these clients, re-engage them with their HIV medical provider and have them retained in HIV medical care.  Our Care Outreach Specialists work closely with local medical care providers and ACSNJ Medical Case Managers to find individuals who are lost-to-care and quickly re-enter them into the HIV health care system.  Clients who have not been to an HIV medical care provider in the past 6 months (or longer) are eligible for Care Outreach services. Our Care Outreach Specialists will assist with scheduling medical appointments, accompany clients to their first 3 appointments and provide transportation.  A referral for this service can be made by contacting the Ray of Hope Coordinator at  (856)338-1344 or click here.
